As a patient you will receive a full evaluation from the Downtown Center’s podiatrists, who determines the location and the nature of the problem and the course of action for further diagnosis and treatment.

For the duration of your visit, all aspects of care will be coordinated. If surgery is indicated, the visit will include a surgical consultation.

Every patient receives a treatment plan created specifically for reduction in pain and improvements for a more active lifestyle.

The New Patient Forms may be filled out, printed and brought into our office. With these Forms you can save time in the office by filling them out now, printing out the forms and bringing them with you to your doctor's appointment. You may also like to print a copy for your personal records.

If you do not have time to complete the forms at one time, we suggest that you print them out and complete the forms at your convenience and bring the completed forms with you when you come in for your appointment.
